Do I need to submit a health form?
All full-time students, including transfer and graduate students must submit a health form to the Office of Student Health Services.
All international students must submit a health form to the Office of Student Health Services.
A student is not considered cleared until all the sections of the health form have been completed.
- Section I: The Report of Medical History is to be completed and signed by the student.
- Section II: The Physical Examination must be completed and signed by a healthcare provider.
- Section III: The TB Risk Assessment must be completed and signed by a healthcare provider.
- Section IV: Immunizations must be completed by a healthcare provider. You may also attach a copy of the immunization record to the health form.
What Immunizations are required?
Two Measles, Mumps, and Rubella (MMR) vaccines or a blood test (titers) to confirm immunity.
- If blood work is done, attach a copy of the lab report to the health form.
- Documentation of the Polio series.

Tetanus-Diphtheria-Pertussis
- Documentation of primary series and booster within the last 10 years.
- A one- time dose of Tdap is recommended to prevent Pertussis (Whooping Cough)

Meningococcal Meningitis Vaccine
- A booster dose is required if the first dose was received prior to 16 years of age.
The Student Health Center offers health care to all full-time residential and commuter undergraduate students (12 credit hours or more) and full-time residential graduate students (6 credit hours or more).
Pre-Entrance Exams
All students must have a pre-entrance physical examination by a Primary Health Care Provider and the results submitted to the Student Health Center on the appropriate form in order to complete the registration process, receive clearance to move into campus housing, and receive heath care services.
Health Insurance
All full-time students are enrolled in the mandatory basic health insurance plan through Maksin (www.studentinsurance.com [3]). Students should also maintain their current private health insurance plan. The University plan is supplemental and is not designed to meet all of your health care needs. The student’s private plan is primary and will be billed first (except for Medicaid plans).
